The Center for Healthy Aging is an outpatient program.  Our goal is to optimize the quality of life for older adults, their caregivers, and family members. Our programs include:

Our mission is to provide consultative evaluation, diagnosis, treatment, and support to individuals with memory concerns. At the Central Jersey Regional Memory Center, anyone with memory concerns has a place to go for early detection, compassionate care, and memory loss education.

Once you have scheduled your Memory Center Assessment appointment, please complete these documents:

  • Memory Center Assessment Questionnaire
  • Authorization for Disclosure of Protected Health Information (print and sign)

These forms need to be completed and returned as soon as possible along with a copy of your insurance cards. The Authorization to Release Protected Health Information can be faxed to 908-788-2525 or dropped off in person.  If there is adequate time prior to your appointment, you can mail via the U.S. Postal Service.

Day of your Appointment

Your appointment will take approximately 2 hours. Please eat prior to your visit. It is your responsibility to check with your insurance company (including Medicare), as to whether pre-authorization or referral is required.

Hunterdon Medical Center is a teaching institution and the Memory Center Assessment Program participates in the teaching of medical residents. Should you object to having a medical resident present at your appointment, please contact us prior to your visit.

Please bring with you:

  • All medications (including supplements and over the counter medications) in their original bottles.
  • The completed forms and questionnaire (if not faxed or mailed previously).
  • All insurance cards.
  • Photo identification.

We encourage you to check with your insuracne company prior to your appointment to verify benefit coverage for neuro-psychological testing and memory testing. Please provide the following codes to your insurance company to verify coverage:

  • 99245 = Office Visit
  • 96138 & 96139 = Testing Codes

A comprehensive evaluation of the older adult by a team of health care professionals specializing in geriatrics (care of the older adult).

Since your health represents a combination of many complex factors, your Geriatric Assessment will gather information about health habits, lifestyle and living arrangements, home safety, and other aspects of your life. Social workers or nurses from the Geriatric Assessment team will examine these issues with you.

A geriatrician (a physician specializing in the care of the elderly) or geriatric nurse practitioner will conduct your physical examination, which includes a complete health history for you and your family.

Following your physical examination and information gathering, the physician may order additional tests, blood work or x-rays. These may be conducted immediately following your examination.

Sometimes patients need to be assessed due to mental status changes observed by family and friends.  It may be noticed that the patient is not keeping up with hygiene, bills, keeping the home clean and eating properly.  This may be due to an illness or perhaps there is a change in the person’s cognition.  There may be times when the person seems confused.  When this happens, it is important to address the concerns and see someone who can do a proper evaluation.  Our geriatrician physicians are very familiar with this assessment and will gladly have an appointment made to address the issue.

Many times an attorney may ask to have a capacity evaluation done if there is concern that the person may not be legally able to sign documents because they are not thinking clearly or they do not understand what is going on.  This is also a situation when a capacity evaluation is valuable.

The Faint and Fall Program is available through Hunterdon Cardiovascular Associates. The program is dedicated to care for patients suffering from faints and falls in the area. Seeing multiple physicians over an extended period of time can cause anxiety, unnecessary tests, and risks of injury or hospitalization while you wait for proper treatment.

The Faint and Fall specialists use recommended guidelines to order only the most appropriate tests for you, resulting in fewer unnecessary procedures, a much shorter time to diagnosis, and increased peace of mind. Our comprehensive approach to the evaluation and treatment of fainting and falling helps to reduce your risk of a faint or fall-related injury and prevent expensive inpatient hospital stays.

If you’ve had an episode of unexplained fainting or falling, you might be concerned that it could happen again at any moment.  There are many possible reasons for fainting and falling, including cardiac conditions, neurological causes, metabolic disorders and even stress.
