Completing the form
- Before completing the document save the form to a location on your computer. (Example: Desktop or Documents).
Note: Do not complete the form online within your web browser; your data will NOT be saved. Please save it to your computer first, and then fill it out.
- Once you have saved the form to your computer, you are ready to complete the form.
- Open the fillable form.
- After you have completed the form, save a final version of the file to your computer by clicking on “save” located at the bottom of the form.
Printing the form
- Once the form is complete, click on “print” located at the bottom of the form.
- Print the form and send it to Occupational Health Services.

If you are new to Occupational Health Services or have changes to information previously provided to us, please complete a Client Registration Form to ensure we have all the required information for your company on file.
A completed Employer Authorization for Treatment Form is required for each employee visit to ensure your employees are provided all necessary services based on your company’s specific needs. This form can be faxed to Occupational Health Services prior to an employee visit or employees can provide this form at the time of their visit.
