Introducing Our New Team of Community Health Workers

We’re excited to share a new resource available to help support our patients.
We are pleased to introduce our new team of Community Health Workers (CHWs), who are dedicated to addressing patients' non-clinical needs that can significantly impact their overall health and well-being.
Our team members are Lauren Brons, Tim Dowd, Lucy Eaton (bilingual), Pamela Hissner, Aidan Morales and Hanna Stutzman.
Our CHWs work with patients to connect them to valuable community resources that address challenges such as:
- Food insecurity
- Housing concerns
- Transportation barriers
- Financial assistance
- Other social and community support needs
