Home Health Services is a non-profit home health agency certified by the federal government, licensed by the New Jersey Department of Health, and accredited by the Joint Commission on Accreditation of Healthcare Organizations.  Hunterdon Health's Home Health only services Hunterdon County. 

Our Team Approach

At Home Health Services, each patient’s program is carefully guided and planned so that all care is integrated for maximum benefit. Our team includes:

  • Home Care Nurses
  • Physical Therapists
  • Occupational Therapists
  • Speech Pathologists
  • Medical Social Workers
  • Home Health Aides

Your physician plays a key role in developing your special home health care program. Your physician also reviews the Plan of Care periodically to ensure that your current healthcare needs are being met.

The Home Health Services staff of nurses, therapists, social workers, and home health aides will work with you to find ways to help you feel better, manage your chronic illness, or recover sooner. They can answer your questions about medication or symptom management, therapeutic regimens, and better nutrition. They are your personal “team”!

As the leader of your care team, you should feel free to participate in decisions about your care, ask questions, and make suggestions. After all, no one knows you as well as you do!

In order to be eligible for admission to Home Health Services, patients must:

  • Be homebound (unable to leave home without assistance)
  • Have ongoing medical supervision and orders from a physician
  • Require periodic visits from at least one of four skilled primary services

1. Skilled nursing

2. Physical therapy

3. Occupational therapy

4. Speech-language therapy

Patients need no prior hospitalization to qualify for home health care.

The frequency of visits and duration of your care is determined by your physician and the professional Home Health staff based on short and long-term patient goals.

If home health care is not indicated, your physician is consulted, and he or she may refer you to another community agency or service.

People of all ages who have experienced illness, injury or disability. If you require help with your health care or activities of daily living (bathing, meal preparation, dressing, etc.), Home Health Services can make a positive difference in your day.

Anyone who is concerned about the health and well-being of a friend or family member may request an assessment. This includes physicians, family members, hospital staff, friends, clergy, and patients themselves.

It may be if…

  • You require continuity of nursing care from hospital to home.
  • You need help returning to your previous function following illness or surgery.
  • You want to avoid unnecessary admission to the hospital or a nursing home for yourself or a family member.

Home Health Services’ care is paid for in many ways:

  • Medicare for those over 65, or on disability, who require highly skilled care.
  • Medicaid for eligible medical assistance through public assistance.
  • Private insurance for those whose insurance provides coverage for home health or private duty nursing care.
  • Private pay for those who do not qualify for insurance-based reimbursement or who have exhausted their reimbursement under other coverage. Fees are based on a sliding scale.

For families or patients interested in-home care, contact the Intake Department at 908-788-2563 to discuss your home care needs.   The Intake Office is open Monday through Friday from 8:00am-4:30pm.

If it is after hours, you may contact the main Home Health Line at 908-788-6138 to speak with an on call nurse.
