If you are interested in having your patient seen by our Home Health Team, you can fax a copy of the last office visit, demographics, and insurance to the Intake Department at 908-788-6681.
If your patient is Medicare primary, regulations state that the patient must be seen by a doctor within 90 days prior or 30 days after the start of home health services that pertain to the reason the patient needs home care services. Medicare requires the physician to complete a face-to-face encounter with documentation of the reason why the patient needs home care and what clinical findings and functional status that causes patient to be home bound.
