Hunterdon County Medication Access Program (HCMAP) is designed to help provide individuals with the prescription medications they need, as well as to educate each person on resources available to assist them in leading a healthier life.  HCMAP can help you find out if you qualify for discounts on your existing medications or if you are eligible for free medications through a patient assistance program.  We will work closely with you and your physician to ensure you receive the maximum amount of assistance for which you qualify.

  • Contact HCMAP for an initial screening and an enrollment form.
  • Once we have received the completed enrollment form we will contact your physician to obtain a list of your prescription medications.
  • We will prepare the application and meet with you to review and sign all necessary applications.  You will be asked to bring financial documentation with you to this meeting.

The entire process may take in excess of several weeks.  Please be prepared to disclose financial information.  You will also be asked to sign an application for each medication you take.  Once approved, HCMAP will place your refills for you.

HCMAP will assist in the application process, but cannot guarantee approval by any individual pharmaceutical company.
